When you shift from one state to another, your name must also be transferred from the electoral roll of your previous assembly constituency to that of your new assembly constituency. A bonafide address proof which could be a Driving License, Passport, Ration card etc. A valid identity proof such as Aadhaar, PAN etc. Two latest passport-sized photographs. A copy of existing Voter ID card.

Visit the official website of National Voters' Services Portal. Click on 'Form 8' link, ie. for 'Correction of entries on Electoral Roll.' Enter the required personal and constituency-related details. Upload the necessary documents, viz. ID proof, address proof, passport-sized photographs, and Voter ID card copies. Finally, verify the information provided by yourself, and submit your request.
How to change Voter ID address via offline mode
Just in case if you are not into technical stuff you can also change the permanent address on your Voter ID card via offline mode. In order to do so, you need to make an errand to your nearest Electoral office. Obtain 'Form 8', fill the required details, and submit the same, along with necessary documents.
